Job Opportunity: Community Occupational Therapist Location: Telford or Shrewsbury Contract: Temporary – 12 weeks Hours: Part-time (22.5 hours per week, flexible – days to be discussed at interview) Working Hours: 09:00 – 17:00 Start Date: ASAP Travel: Must be a car driver with access to own vehicle (mileage paid at 40p per mile) Registration: HCPC Registered We are looking for a dedicated and compassionate Community Occupational Therapist to join our Dementia Team on a temporary, part-time basis. This is a fantastic opportunity to make a real difference in the lives of individuals living with dementia, providing essential support within a fast-paced and rewarding environment. Key Responsibilities: Conduct specialist Occupational Therapy assessments and interventions within service users’ homes. Work as part of a multi-disciplinary team (MDT), providing specialist consultative knowledge and support. Manage a caseload of service users, balancing generic and profession-specific responsibilities, including care coordination. Undertake team duty roles, contributing to the smooth operation of the service. Liaise effectively with a range of stakeholders, families, carers, and local community services through written and verbal communication. Collaborate with other agencies to develop and implement individualised care plans. Work autonomously within the multidisciplinary team, with supervision from the Clinical Lead and support from specialist Occupational Therapists. What We Are Looking For: Previous experience in a community setting, with the ability to work with individuals in their own homes. Mental health experience (previous experience in dementia services is highly desirable). Strong communication and problem-solving skills, with an innovative approach to patient care. Ability to work flexibly, adapting to the dynamic needs of the service and its users. A passion for making a positive impact in the lives of individuals and their families.
