Social Worker - Adults - 2 YRS PQE (Grade I-K)

Who You Are You are a dedicated and committed social worker experienced in working within a multidisciplinary team environment. You possess a strong belief in promoting independence and have a keen focus on working in a strengths-based, outcome-oriented manner. You have post-qualification experience and a solid understanding of relevant legislation, particularly concerning the safeguarding of vulnerable adults. What the Job Involves This role is positioned within the Reablement Service, a dynamic team led by occupational therapy and focused on rehabilitation and enablement. The primary goal is to support residents to achieve the highest level of independence possible, often in collaboration with the Admission Avoidance Discharge Service (AADS) to explore more integrated ways of working. You will manage various cases, conducting assessments that emphasize the individual's strengths and their support systems to enhance independence. The position requires you to complete Care Act Assessments, develop support plans, and address safeguarding concerns as part of a multidisciplinary team. Skills Experience in social work within a multidisciplinary team Strong focus on strengths-based, outcome-oriented approaches Excellent ability to assess and promote independence in individuals Proficient in managing diverse caseloads In-depth knowledge of relevant safeguarding legislation and responsibilities
