Adult Social Worker - 2- 4 years (Grade H)

Who You Are We are looking for a qualified, skilled, and committed Social Worker with experience in adult social care to join our Social Care Direct team. You should be experienced in working with individuals in a strength-based approach, with at least 4 years of experience. The ideal candidate will have worked in environments offering short-term interventions and thrive in fast-paced settings, while also being capable of identifying the presenting problem early to offer focused and effective intervention. IT literacy is essential, and experience with Mosaic is preferred. What the Job Involves As a Social Worker in our Social Care Direct team, you will be the first point of contact for all initial enquiries about social care. You will focus on placing individuals at the heart of assessments, involving their families, friends, and communities to help improve their lives and achieve their goals. Your responsibilities will include assessing and reviewing care support needs, providing information, advice, and care to help adults regain or maintain independence, and upholding a high standard of social work in accordance with legal and policy requirements. You will conduct complex assessments of care needs, risks, and mental capacity, and determine Care Act eligibility thresholds.
