This exciting, innovative and challenging role may be for you The Assertive Community Clinician in Child and Adolescent Mental Health (CAMHS) will provide extended hours consultancy, clinical coordination, advice and assessment where appropriate to clients under 18 presenting in a number of different settings across STG and TSH catchment area, experiencing acute or complex mental health concerns with the aim of reducing hospital presentations and admissions. The Assertive Community Clinician is required to deliver assertive outreach, providing specialist mental health services and tertiary consultation to children and young people in community settings including schools, youth accommodations, homes and other community settings. The Assertive outreach clinician will also provide this service within a CAMHS service. The Assertive Community Clinician in CAMHS will facilitate, implement and evaluate care management pathways for patients with complex health needs who are at risk of admission to hospital.
