Job Title: Social Worker Job Specialty: Social Worker - Hospice Job Duration: 13 weeks Shift: 8H Days, 8:00 AM - 4:30 PM Guaranteed Hours: 32 hours per week Experience: Minimum of 2 years License: Clinical Social Work License in the state of Oregon required Certifications: BLS, LCSW (Licensed Clinical Social Worker) Must-Have: Hospice experience Job Description: - Conduct psychosocial assessments and determine patient and family needs. - Develop a plan of treatment consistent with identified needs. - Act as a consultant to other disciplines and interpret psychosocial information to patients, families, and other professionals. - Work to connect patients and their families to community support systems. - Provide outpatient and hospice care. - Participate in department-specific orientation. - Use Pointcare (HCHB) for electronic medical record-keeping. - Work schedule includes weekdays, no weekends or holidays required. - Dress code: Business casual.
