Home Health Clinical Manager - Job Description Job Summary: The Clinical Manager has a broader leadership role that involves both clinical oversight and administrative duties. This role ensures that the home health agency runs efficiently, maintains compliance with regulations, and delivers high-quality care. Key Responsibilities: Oversee the entire patient care program, ensuring effective delivery of services. Assign and coordinate patient care services and personnel scheduling. Manage referrals and intake processes, ensuring smooth transitions for patients. Ensure continuous assessment of patient needs and the development of individualized care plans. Participate in hiring, evaluating, and training staff. Maintain compliance with state and federal regulations, including Medicare/Medicaid guidelines. Implement and monitor quality assurance programs to improve patient outcomes. Act as a liaison between administration, physicians, and clinical staff. Ensure sufficient coverage during operational hours and designate a backup when off duty. Provide Admissions when necessary Qualifications: Registered Nurse (RN) or other licensed healthcare professional (PT, OT, SLP, etc.). 3+ years of home health experience, with at least 1 year in a leadership role. Strong leadership, organizational, and regulatory compliance skills.

Compensation details: 85000-95000 Yearly Salary
