SUMMARY
Family Service League is seeking a full-time Case Manager for our Children’s Care Coordination Program in Bay Shore, NY. The Children’s Care Coordination Program is a home visiting program that offers care coordination for children and young adults with mental illness and/or chronic medical conditions. The Case Manager will be responsible for conducting home visits/meetings (both in-person and telehealth) with clients to develop strength assessments and individualized planning, providing linkages, assisting with navigating service systems, and supporting them with their goal achievements.
Evening hours are required for at least 2 days a week. Travel is required.

RESPONSIBILITIES
- The Case Manager will outreach and engage clients with a primary history of mental illness.
- Develop Strength’s Assessment with the client.
- Develop crisis/safety planning, in collaboration with the Mental health providers.
- Develop Individualized planning with the client in the areas of health, education, mental health, vocations, socialization, benefit, legal and life skills.
- The Case Manager will meet a minimum of two (2) times a month to support the youth with their goal achievements.
- Advocate for clients and assist in the coordination and linkage of services with providers, benefit systems and the community.
- Help clients navigate service systems to get the support that they need in their recovery.
- The Case Manager will report to the agency all incidents and accidents immediately.
- Inform agency Supervisors/Director when other providers are not collaborating in the effort to achieve services for the client or have plans to inappropriately discharge a client from housing.
- Follow guidelines set by the Office of Mental Health and the Division of Mental Hygiene CSPOA unit Risk Management.
- Work within a team approach with Family Peer Support and youth Peer Support workers. As well as all providers of the child/family treatment teams.
- All other duties as assigned.
QUALIFICATIONS
Bachelor’s degree in social work and 2 years of relevant experience required OR Master’s degree in social work and 1 year of relevant experience required.
Excellent interpersonal, verbal, and written communication skills required.
Proficient in computer skills, including Microsoft Office required.
Ability to work individually and as part of a team required.
Ability to multi-task and work in a fast-paced environment required.
Valid and clean NYS Driver’s License.
PHYSICAL REQUIREMENTS
This position requires sitting for long periods of time and travelling within the community to conduct home visits.
