Locum Physician (MD/DO) - Palliative Medicine in New York

Job Title: Hospice Physician Job Specialty: Hospice Job Duration: 13 months Shift: Days, 8:00 am - 4:30 pm, Monday to Friday Guaranteed Hours: Not specified Experience: Not specified License: New York State License required Certifications: BLS and Board Certified in Internal Medicine, Family Medicine, or Emergency Medicine required Must-Have: Local residency Job Description: - Collaborate with interdisciplinary team meetings to review cases for hospice eligibility and provide guidance and recommendations to field nurses. - Conduct home visits for face-to-face encounters and symptom management as needed, with an average of 0-2 visits per day. - Participate in an on-call schedule with an average of 1 weeknight per week from 4:00 pm - 8:00 am and 1 in 4/5 weekends from 4:00 pm on Friday until 8:00 am on Monday. - Utilize the EPIC Comfort/Hospice Application for documentation. - Fellowship training in Hospice and Palliative Medicine or Geriatrics is a plus but not required. - Provide care coverage due to retirement and home care needs.
