Job Title: Occupational Therapist Length of assignment: 12 Weeks Hours per week: 37 hours Hourly rate:£29.99 Location: Walsall Job Information:To work in a MDT team, to complete initial and subsequent reviews of packages of care at home following discharge from hospital. To use a strength based approach to complete OT assessment/reviews, moving and handling assessments and single handed care assessments, to promote a citizens independence at home. Assess for and provide equipment, assistive technology and signposting to community assets and to determine if any, long term support is needed. To complete revised support plans if there are changes to care and risk assessments when required.
