Occupational Therapist Community North-West Melbourne Full and Part-time

Lunare Healthcare are searching for an enthusiastic and motivated Occupational Therapist to join our team. Full or part time roles available working in community care to deliver services to clients on Home Care Packages (HCP) or Short Term Restorative Care (STRC) Programmes across the North-Western suburbs of Melbourne, VIC. This role requires a passionate individual who has empathy and drive to deliver the best service and care to the aged care community. New graduates are welcome to apply . What’s on offer: Generous pay rates paid travel Flexible working hours CPD allowance provided Opportunity to work from home Support and mentoring Phone and tablet provided Duties & Responsibilities : Home assessments for a range of clients from a variety of different Aged Care Package providers. Delivering evidence based Occupational Therapy services. Assessment and prescription of assistive equipment. Assessment and planning of home modifications aligned with Australian Standards to improve client independence and safety. Comprehensive written reports (templates provided). Assessment of mobility and function and a prescription for the provision of appropriate mobility and functional equipment. Liaise with families, specialist services, case managers and other stakeholders as appropriate. Skills and Experience / About You AHPRA registered Occupational Therapist Tertiary qualification in Occupational Therapist Full working rights in Australia Experience in community-based occupational therapy or aged care desirable but not essential.
