Job Title: Band 6 CPN Care Coordinator – Early Intervention Team Tripod Partners are currently seeking a skilled Band 6 Community Psychiatric Nurse (CPN) to join the Essex Support & Treatment in Early Psychosis (ESTEP) team. This role is based at Laurel House, Rochford Hospital, and focuses on providing care coordination for early intervention in psychosis. Key Responsibilities: Act as a Care Coordinator for individuals within the Early Intervention pathway. Deliver comprehensive mental health assessments and care planning. Provide therapeutic interventions tailored to patient needs. Work collaboratively with a multi-disciplinary team to ensure seamless patient care. Position Details: Location: Rochford, Essex Work Pattern: Monday to Friday, 9:00 AM – 5:00 PM Start Date: 3rd February 2025 End Date: 3rd April 2025 Rate of Pay: £26.87 per hour Pay Type: Umbrella or PAYE available Requirements: Registered CPN with experience in mental health care coordination. Previous experience working within Early Intervention or similar services. Strong skills in mental health assessment, care planning, and delivering interventions.
