Hospital Discharge Social Worker - £37.46

Winlight Recruitment are currently seeking an experienced Social Worker to join the Hospital Discharge Team at Pilgrim Hospital in Boston, Lincolnshire. This is a fantastic opportunity to be part of a dedicated team supporting patients through the discharge process, ensuring they receive appropriate care and support once leaving the hospital. Key Responsibilities: Support patients in the discharge process, ensuring a smooth transition from hospital to home or alternative care settings. Complete comprehensive assessments for patients, identifying their needs and developing tailored care plans. Collaborate with healthcare professionals, families, and community resources to arrange appropriate post-discharge care. Liaise with external agencies to ensure the necessary support services are in place for patients. Maintain accurate records and documentation in compliance with hospital policies and procedures. Work closely with the multidisciplinary team to provide holistic and person-centred care to patients. Requirements: Qualified Social Worker with SWE registration. Proven experience in hospital discharge and working within a healthcare setting. Strong communication skills and the ability to work effectively as part of a team. Ability to work autonomously and manage your caseload effectively. A commitment to providing high-quality, person-centred care. Full-time availability to work on-site 5 days per week (37 hours). Driving License and access to a vehicle (preferred but not essential)
