The Care Manager will be responsible for case management, training, elder care, social services, and medicine. Day-to-day tasks include coordinating care plans, providing training to staff, overseeing social service initiatives, and ensuring the well-being of elderly clients. Qualifications Case Management and Elder Care skills Training and Social Services expertise Knowledge of Medicine in a care setting Strong communication and interpersonal skills Ability to work collaboratively in a team Experience in care management or related field Relevant certification or qualification in health or social care
