Key Responsibilities : Conduct urgent assessments for patients in community settings. Develop and implement individualized treatment plans to promote independence. Collaborate with the multidisciplinary team to coordinate care effectively. Support patients with complex needs to avoid hospital admissions. Key Requirements : HCPC registration as an Occupational Therapist. Minimum of 6 months NHS experience in community or rapid response settings. Strong clinical decision-making and communication skills. Ability to work independently in a fast-paced environment.
