Part-Time Leaving Care Personal Advisor
Ashton-under-Lyne, Tameside
06/01/2024 – 1 year initially
21 hours per week over 3 days – Hybrid
Supporting young people within their home or placement
Attend, plan and carry out reviews and home visits
Make face to face contact with young people referred to the project, and work to engage them with the service
Undertake support needs analyses with young people, and develop individual plans to address their future aims and objectives
Follow and implement recommendations of reviews and Pathway Plans
Work with the young people to help reach their goals
Support to access positive activities, education, training, employment, housing, health support
Direct work with others who are relevant in their lives
Develop supportive and empowering relationships with young people to build their self-esteem, confidence, knowledge and resilience to enable them to move forward positively in their lives
Respond appropriately to any child protection/Adult at Risk concerns as and when they arise
Liaising with other agencies on their behalf
Keep and maintain records of work done
Attend meetings, reviews and forums relating to the young people where necessary Requirements
Full UK Driving Licence, access to a vehicle, business insurance
Minimum of a NVQ Level 3 Care of Children and Young People
Experience of working with young people, vulnerability and complex needs, behaviour needs & independence skills
Experience in assessment of children’s needs
Excellent organisation skills
