The Annual Wellness Visit (AWV) Nurse works collaboratively with the primary care provider, care coordination team, and all members of the healthcare team. AWV Nurse works to identify Medicare patients who are eligible for an AWV and executes a guided process to complete the AWV under the direct supervision of the patient’s primary care provider. May also assist Quality Department and Care Coordination Team with completion of other tasks as needed. This position requires a candidate who can effectively work with a diverse group of patients and stakeholders within the healthcare setting.



Primary Responsibilities:



  • Will participate and maintain a culture within The Corvallis Clinic that is consistent with the content outlined in the Service and Behavioral Standards Handbook. To this end, employee will be expected to read, have familiarity, and embrace the principles contained within

  • Engage and empower patients to participate in the preventative care approach

  • Identify when patients are eligible for AWV that are on the schedule and AWV gap lists

  • Work collaboratively with clinical teams to accomplish AWV in an efficient manner for the patient and care team

  • Follow AWV protocols and standing orders including completion of preventative services that may result from AWV

  • Develop and share knowledge of AWV eligibility and documentation requirements

  • Address open quality gaps with patients and work collaboratively with patients toward the education of these gaps and closure of them

  • Appropriately document within the electronic medical record (EMR). Support maintaining accurate and up to date information in the (EMR) by gathering consult notes, diagnoses list, and medication lists.

  • Utilize appropriate protocols and standing orders in delivering patient care

  • Participate in regular team meetings, huddles, staff meetings, and quality improvement projects to improve patient care delivery

  • Consult with clinical staff and ancillary department staff to reduce barriers to the efficient delivery of care. Identify service delivery problems and potential for effective patient management intervention

  • Maintain clinical competencies for role

  • May assist with other clinical duties in the Quality Department and Care Coordination team, as needed

Required Qualifications:



  • Current Oregon Licensed Practical Nurse or Registered Nurse required

  • Current Basic Life Support (BLS) certification or ability to complete BLS certification within 90 days of hire required.

  • Proficient user of computer: Microsoft programs and Electronic Medical Records



Preferred Qualifications:



  • Experience in the following preferred: Nursing experience in primary care, community nursing, critical care, or home health

  • Maintain patient, team member, and employer confidentiality - comply with all HIPAA regulations

  • Understanding and appreciation of Social Determinants of Health (SDoH) and Preventative Medicine

  • Proven awareness of scope of practice boundaries, comfortable seeking direction and assistance from appropriate resources

  • Proven ability to work independently and exercise clinical judgment in interactions with providers, patients, and their support person(s)

  • Proven solid organizational and time management skills, evidenced by a capacity to prioritize multiple tasks and role components

  • Demonstrated initiative, ability to work with others in an engaging manner, and good professional judgment

  • Proven solid team player with passion for interdepartmental collaboration

  • Proven to display a positive attitude and act as a positive agent for change
