Pay Rates:
$18.03- Residential programs
$19.95- Outpatient programs

What will you do? As a Case Manager at Aspire Health Partners, your job purpose will be to provide a variety of essential functions to ensure effective operations of assigned programs.

These functions include:

  • Providing screening, referral, emergency, crisis and case management services, participating in clinical treatment team.
  • Conducting intake assessments to determine family needs, efficiently documenting all activities, conducting activities with the client and families, providing regular input to the treatment plan.
  • Attending community meetings to advocate for client needs, providing support and crisis intervention when necessary.
  • Documenting clinical records and additional program specific documents while maintaining the integrity of the client.
  • Ability to use program technology to communicate effectively with staff.
  • On occasion, act as the authorized client transporter for necessary treatment and services.

Qualifications

  • Bachelor's Degree in Social Work, Psychology or Human Services related field.
  • Pass a Level II Background clearance.
  • Pass a Motor Vehicle Records review.
