SUMMARY
The Medical Director is a licensed primary care physician and provides medical management of patients at Sulzbacher Clinics and in the field on outreach vehicles to homeless individuals. The Medical Director oversees policies and procedures, protocols and quality measures in the Sulzbacher Adult Clinics. The Physician actively collaborates with other healthcare providers to ensure holistic and integrated patient care, and maintains documentation and patient records in accordance with regulatory and Center policies. Sulzbacher Adult Medical Clinics do not prescribe controlled substances.

SUPERVISORY RESPONSIBILITIES

ESSENTIAL DUTIES AND RESPONSIBILITIES
- Develop and implement treatment plans based on clinical experience and knowledge. Refer patients, clients, or family members to community resources or to specialists as necessary.
- Serves as a key collaborator for the Mayo Learner's Clinic.
- Complete all relevant charting, documentation and follow-up. Fill out and maintain client-related paperwork, including federal- and state-mandated forms, client diagnostic records, and progress notes.
- Communicate with families, probation officers, police, or other interested parties to exchange necessary information during the treatment process.
- Evaluate the effectiveness of individualized treatment plans on patients' progress in resolving identified problems and moving towards defined objectives.
- Coordinate patient care with members of the Housing Team and other Sulzbacher Clinic departments with a particular focus on integrated care.
- Perform Medical Director responsibilities including general oversight of policies, protocols and compliance for internal medicine/family medicine providers. Anticipate this role to require approximately 2 hours per week outside of direct patient care.
- Serve as a resource to mid-level internal medicine/family practice providers.
- Participate in all appropriate team meetings and required training sessions.
- Maintain active, unrestricted license.
- Be comfortable working with individuals living on the street or in the woods as well as in a clinic setting.
- Participate with local and regional community partners to identify best practices and collaborate to combat HIV.
- Engage new and existing patients in HIV prevention services, identifying those at risk for HIV using validated screening tools. Increase the number of patients tested for HIV. For those who test negative for HIV, provide HIV prevention education, and prescribe and support the use of clinically indicated PrEP. For those who test positive for HIV, link them to HIV treatment.
- Participate in all trainings as required.

The Medical Director is sensitive to cultural, spiritual and practical needs of patients, residents, families, guests and staff members in all interactions. He/she provides positive feedback and comments about the quality of work and effort that have been offered by patients, residents, families and coworkers, and adjusts priorities and schedules to meet specific customer needs. The Medical Director reports and/or corrects problems related to safety, cleanliness and comfort of resident, patient and work areas, and follows Universal Precautions and Infection Control techniques. The position demonstrates an understanding of their role in facility safety plans and procedures related to fire, weather and other disasters.
EDUCATION and/or EXPERIENCE
- MD/DO unrestricted license to practice in the state of Florida
- Minimum 2 years' experience prescribing in a primary care setting
- Board certification in family or internal medicine preferred
- Excellent interpersonal and time management skills
- Customer service focused
- Excellent written and oral communication skills
- Precepting experience preferred and willing to take students
- Prior street outreach experience highly preferred
- Infectious disease background highly preferred
PHYSICAL / MENTAL DEMANDS & WORKING CONDITIONS
- Transportation and valid Florida driver's license
- Ability to pass a Level II Background Screening
- Physically able to work in a fast-paced environment
