Job Description


Description:

The Licensed Practical Nurse will assist with the comprehensive delivery of personal care services to residents on a day-to-day basis following required regulatory requirements. The Licensed Practical Nurse will also assist in maintaining a physical, social and psychological environment, which will be conducive to the best interests and welfare of the residents.


Duties & Responsibilities:

  • Ensures compliance with all federal, state and local employment laws.
  • Servant Leadership.
  • Conducts resident evaluations in a prompt and timely matter according to state regulations and LAHP policies and procedures.
  • Oversees and is responsible for all aspects of medication, including but not limited to; orders, administration, storage, documentation and as mandated by the State Regulations and Guidelines as well as Company policy and procedure.
  • Assure that all resident care specialists are familiar with the residents' individual conditions and are trained to provide the resident services in an effective, professional, and sensitive manner.
  • Monitor's resident wellness care for compliance with state and federal regulations
  • Monitors and maintains vital statistics as required.
  • Ensures in house ancillary services, such as x-rays, podiatry, dental, eye care, psych visits etc. are scheduled and the appropriate follow up is completed.
  • Respond in a timely manner to requests of residents, families, and guests' programs.
  • Performs other related duties as assigned.


Requirements:

Required Skills & Abilities:

  • Able to supervise department employees using independent judgment and discretion.
  • Must be able to communicate effectively with residents, families, staff, community and State officials and general public.
  • Must have compassion for and desire to work with the elderly.
  • Ability to develop systems for monitoring resident health conditions and for ensuring efficient and safe delivery of resident care.
  • Must demonstrate the ability to work responsibly as a team member as well as an individual.
  • Must have exceptional organizational skills and ability to re-prioritize daily tasks in order to accommodate fluctuating needs of the resident and Community.
  • Must practice and promote LAHP Policies and Procedures, Mission Statement, Core Values and Founding Principles.
  • Must have strong documentation skills.
  • Strong leadership skills.
  • Able to make independent decisions.
  • Able to communicate effectively with all levels of management, team members, residents, family members, guests, vendors, referral sources, and outside contacts.
  • Must be able to communicate in a warm, friendly and caring manner.
  • Able to concentrate with frequent interruptions.
  • Able to work under stress and in emergency situations.
  • Able to talk and hear effectively to convey instructions and information to residents and team members.
  • Must meet all health requirements and pass background checks.
  • Must have valid driver's license.

Education & Experience:

  • Licensed Practical Nurse or Registered Nurse with active license required.
  • Minimum Two (2) years in the senior living environment
  • Licensed LPN/RN in good standing with State Licensing Authority.
  • Memory care experience preferred.
  • Knowledge of State and local rules, regulations, laws and ordinances relating to licensed senior housing.

Physical Requirements:

  • While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; talk or hear. The employee must occasionally lift or move office products and supplies, up to 20 pounds.
  • Able to stand or walk 75% of the day.
  • Subject to reactions from dust, disinfectants and cleaning chemicals and may be exposed to infectious waste, diseases and other conditions.
  • Use personal protective equipment and supplies when needed to prevent burns, falls, and infection.
