EOE Position Information: The Care Coordinator RN is assigned to a specific clinical care area facilitating the plan for inpatient/outpatient care by using a collaborative interdisciplinary approach. The Care Coordinator RN will focus on anticipation integration and coordination of clinical care and discharge planning services necessary to meet the needs of patients and to optimize clinical and resource outcomes. The Care Coordinator RN collaborates with the interdisciplinary team in assessing patient needs and developing the plan of care. The Care Coordinator RN will also act as an advocate for the patient physician and hospital relative to compliance with federal state and third party utilization regulatory requirements. Experience Requirements: 3 Years Nursing - Related Area Required - And 3 Years Clinical - Case Management Or 3 Years Clinical - Utilization Management Or 3 Years Clinical - Home or Community Health Preferred Education Required: Graduate Nursing Required Bachelor's Degree Nursing Preferred Licensure/Certification Requirements: Registered Nurse Required Hours of Work: 8:00 am- 4:30 pm Weekend Requirements: As Needed Call Requirements: None
