Job Summary:

• As a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.
• Patient care is delivered within organizational philosophy, policy and standards of community health practice.
• Also required to perform admission processes and case management responsibilities.
• Perform other job duties as needed or assigned by the client.

Requirements:

Active RN license for MA State
AHA – BLS
Must be covid vaccinated
