Your ability to conduct initial and ongoing assessments will help identify clients' medical needs, levels of functioning, and communication skills. Being a proactive problem solver, you will initiate and adjust the plan of care, performing OASIS assessments at key moments to ensure optimal client health.Effective communication is essential, as you'll coordinate services while keeping physicians and other personnel informed about any changes in clients' conditions. Your empathetic nature will shine as you collaborate with patients, families, and other healthcare providers, working together on care and discharge planning. Plus, your organizational skills will be invaluable when it comes to preparing clinical progress notes and submitting paperwork in a timely fashion, all while maintaining integrity and excellence in the home care environment.Knowledge and skills required for the position are:RN provides initial and ongoing assessments to identify the client's medical needs level of functioning and ability to communicate and learn.RN initiates the plan of care necessary revisions and performs OASIS assessments at appropriate time points when applicable.RN Initiates and performs appropriate nursing interventions.RN coordinates services informs the physician and other personnel of changes in the client's condition and needs.RN collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.RN prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.Join our team today!If you think this job is a fit for what you are looking for, great!
