We are currently looking for a Physical Therapist to join our team. The position would be responsible for providing physical therapy evaluation and treatment to patients in their homes or alternate community settings.

Responsibilities:
Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workersAssesses and evaluates client for physical therapy needs and develops and implements appropriate plan of action in collaboration with the physicianDocuments all changes in client’s condition and updates care plans and outcomes accordinglyIdentifies counseling needs and plans appropriate teaching strategiesMonitors quality and appropriateness of services providedAdheres to agency infection control and safety policiesAdheres to the organization’s policy in regards to absenteeism and appearanceOmission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

Qualifications:
Physical Therapy Licensure in NJ required2-3 years of prior clinical experience and community based experience preferredStrong computer skillsStrong communication and interpersonal skillsValid NJ drivers license

Working Conditions/Physical Demand: Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation
