Reablement Officer (Scale 5)

Who You Are

• Possess a Social Care qualification (NVQ/QCF 3 in Social Care or equivalent).


• Minimum of 2 years' experience in supporting vulnerable adults with home and community-based care and support.


• Competent with IT systems and maintaining electronic records in accordance with statutory requirements.


• Ability to review and assess service users to ensure their reablement goals are being met.


• Hold a full clean driving license and access to a vehicle for travel.


• Flexible and able to manage workload independently and as part of a team.


• Polite and courteous when conversing with the public, with the ability to converse in accurate spoken English.


• Preferred: Trusted Assessor trained in the provision of aids and adaptations.

What the Job Involves

• Providing high-quality support to service users, empowering them to achieve their care and health outcomes.


• Working closely with the therapy team to implement reablement plans.


• Assessing, designing, planning, implementing, and supporting care plans for service users.


• Developing and sustaining effective working relationships with service users.


• Providing practical assistance and support to meet the needs of service users as part of a team.


• Ensuring service users have appropriate access to advocacy and support inclusion and participation through various interventions.


• Undertaking relevant training and contributing to team workshops to promote good practice.


• Collaborating with other professionals, carers, and agencies to support service users and their families.


• Adhering to departmental policies, procedures, and statutory requirements to ensure effective service delivery.


• Complying with financial regulations and data management policies, and maintaining accurate information.


• Working on a 4-week rolling rota, including one weekend in four, with flexibility for emergency changes.
