Eligibility Criteria: Certificate of Clinical Competence, must supervise an SLPA, BLS - AHA; Covid with Booster, IL state License

Resume - Certificate of Clinical Competence

BLS- required from AHA

Active IL state license

2 references (within the last 1 year, at least 1 supervisor reference).
