Lead Support Worker

Penzance (Roscadghill) also work in the community in the old Penwith area from Hayle down to Lands End.

Permanent, full time (37.5 hpw)

Pay £12.61 to £13.36 per hour (£24,664 to £26,123 per annum) and great benefits including Health Cash Plan

As our Lead Support Worker, youll be at the heart of supporting our customers who have complex needs to develop their skills to live more independently in the community. Typical day as a Lead Support Worker (known by us as Coordinator)

  • Creating support plans with your customers and working with other Lead Support Workers to coordinate our small team of Support Workers working to the plans.
  • Our customers needs include Mental Health/emotional wellbeing needs and substance abuse issues. You may also work with customers with an acquired brain injury and those with high-risk behaviours, such as hoarding and self-neglect
  • Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
  • Carrying out risk assessments, support planning, goal setting and regular reviews.
  • Cuppas, chats and catch ups. Theres no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring

  • Passion to support our customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating person-centred support plans and supporting colleagues in working to the plans.
  • Experience of coordinating and assessing customer referrals.
  • The ability to workon your own initiative, remaincalm under pressure and havea resilient approach
  • To get from A to B during your working day youll need access to a vehicle, If you have a car etc. it will need to be insured for business purposes. The great news is that well pay your mileage (yes even 20p per mile for Bikes)!

Our team

Youll join Eddy who has been with Home Group for 27 years, (so we must be doing something right), and our Empowering Independence West Cornwall (EI West) team, were super passionate about what we do! Were really proud of the support we give to customers to reach their goals and about celebrating their achievements and numerous successful outcomes. Were a supportive bunch and like to be outdoors! whether thats having a BBQ in the garden with customers, taking a lunch time walk (our office is right on the edge of the countryside), swimming in the sea and walking our dogs before or after work or the occasional healthy walk along the coast organized by our Health coach.

Job details

  • Flexible working hours (or were open to agreeing a work pattern with you)Where were flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
  • Some weekend work may be required on a rotated basis, but we ask for volunteers first!.
  • After your probationary period you will have the opportunity to become an out of hours On-Call responder (paid at £50 for the first hour or part of the hour then at your usual rate of pay should the call out take longer).
  • Although youll lone work, we do get together regularly for training and team meetings.
  • Able to use technology to update support plans, complete online learning and to collaborate with colleagues.
  • Youll need an Enhanced DBS check done and we pay for that.
