Medica Project Outreach Coordinator (Co-Occurring Case Manager)

4-Day Work Week Option Available

$2,000 SIGNING BONUS

Location: Fridley

Starting Pay: $55,068.00 Base salary with potential increases depending on education and experience; mileage reimbursement.

Main Duties and Responsibilities:

  • Providing services to clients using models of treatment that include Person Centered care, Motivational Interviewing, Harm Reduction, and Stages of Change.
  • Provide individual services that address a variety of needs determined by the unique needs or the clients served (i.e., family work, vocational, housing, benefits assistance, and other case management like functions).
  • Provide outreach, monitoring, transition services and assistance with needs as clinically indicated to client in their homes or in other community settings, including driving clients in their vehicle as needed.
  • Communicate with relevant professional and natural support to ensure services are well coordinated.
  • Ensure the primary care/medical needs of the clients are well-coordinated and address the physical needs of the client.
  • Complete all required written documentation according to program policy/regulation.
  • Advocate on behalf of each client to assure the protection of client rights and privacy, as well as assure that clients understand the complaint and grievance procedures.
  • Treat all clients with dignity and respect, providing services that are respectful of age, gender, and culture.
  • Assure that all changes in client status or needs are communicated promptly to the team to enhance continuity of care.
  • In case of illness or client crisis, provide coverage for team members to assure and maintain continuity of care for each client served.
  • Participate in all scheduled staff meetings and trainings, unless excused by your immediate supervisor.
  • Act in a manner to assure client confidentiality and privacy that complies with MHR policies and procedures.
  • Assure that all MHR documentation is completed on time and within MHR policies and procedures.
  • Assure that MHR productivity standards (i.e. Assertive Outreach, case load, face-to-face contact, contact frequency,) are met, as outlined by the Team Leader.
  • Chart maintenance – ensure chart information is up to date (i.e. SUD ROI, Vulnerable adult ROI, status changes, contacts, etc.).

Minimum Education and Experience

  • Bachelor's degree in social work, psychology or human services related field. At least four years of experience working with individuals struggling with co-occurring disorder diagnoses.
