As a Permanent Patient Care Coordinator, you will play an integral role in our manufacturing industry team. This is a temp-to-hire position that requires a minimum of 2 years of experience. The successful candidate will be responsible for coordinating patient care activities and ensuring that all patient needs are met in a timely and efficient manner. This role will involve working closely with other healthcare professionals, delivering high-quality patient care services, and maintaining accurate patient records. The candidate will also be responsible for ensuring compliance with all relevant healthcare regulations and standards.

Responsibilities:
1. Coordinate all patient care activities, including scheduling appointments, managing patient records, and ensuring that all patient needs are met.
2. Work closely with other healthcare professionals to deliver high-quality patient care services.
3. Maintain accurate and up-to-date patient records.
4. Ensure compliance with all relevant healthcare regulations and standards.
5. Handle patient queries and concerns in a professional and empathetic manner.
6. Collaborate with the healthcare team to develop and implement patient care plans.
7. Facilitate communication between patients, family members, and healthcare professionals.
8. Participate in patient care meetings and provide updates on patient progress.
9. Identify opportunities for improving patient care services and make recommendations accordingly.
10. Coordinate patient discharge processes and provide post-discharge support as necessary.

Qualifications:
1. A minimum of 5 years of experience in a patient care coordination role or a similar position.
2. Strong computer skills, including proficiency in using healthcare management systems and Microsoft Office Suite.
3. Excellent communication and interpersonal skills, with the ability to interact effectively with patients, family members, and healthcare professionals.
4. Strong organizational and multitasking skills, with the ability to manage multiple patient cases simultaneously.
5. Knowledge of healthcare regulations and standards.
6. Ability to work effectively in a team, as well as independently.
7. Strong problem-solving skills, with the ability to make effective decisions under pressure.
8. High level of empathy and understanding, with the ability to provide compassionate patient care.
9. Ability to maintain patient confidentiality and handle sensitive information with discretion.
10. Bachelor's degree in healthcare administration, nursing, or a related field is preferred.
