Job Details

The Patient Navigator supports newly diagnosed cancer patients by guiding them through timely, patient-centered care. The Navigator builds trust, coordinates appointments, and maintains close communication with the patient's care team, addressing individual needs to ensure smooth, efficient access to high-quality cancer care, especially for underserved communities.

Responsibilities:

Assist with scheduling and completing appointments
Identify and help remove barriers to timely treatment, such as transportation or childcare
Conduct supportive care surveys to assess patients’ symptoms, financial concerns, and care needs
Offer support with insurance, appointment reminders, and navigation within the Cancer Center
Connect patients with psychosocial, palliative care, and other supportive services
Screen for mental health history and refer patients to social work, psychiatry, or palliative care as needed
Act as a liaison between oncology and primary care teams for seamless communication
The Patient Navigator may also participate in other program-related activities as needed.
