Providence St. Joseph Health is calling an Inpatient Care Manager, RN to work onsite at our St. Luke's Rehabilitation Institute in Spokane, WA.

The Internal Case Manager for Inpatient Rehabilitation, in collaboration with the multi-disciplinary team, coordinates the patient stay and discharge planning from the time of admission to the time of discharge. The Internal Case Manager supports the delivery of quality care in the most cost effective way by managing the human, material and financial resources needed for individual healthcare needs. The Internal Case Manager is also responsible for ensuring patient and patient's caregiver(s) education in preparation for discharge readiness. All employees are expected to be pleasant, respectful and courteous in all interactions with patients, families, staff, and visitors of the Institute, as well as being a positive representative of the hospital at all times.

Required Qualifications:

  • Washington Registered Nurse License upon hire.
  • 3 years - Clinical experience.

Preferred Qualifications:

  • Bachelor's Degree - Nursing.
  • CRRN and/or CCM certifications upon hire.
  • Experience in case management; clinical nursing or utilization management.
  • Experience in the rehabilitation of chronically or catastrophically ill or injured individual.
